Refer-to-Quit Program – as easy as 1-2-3:

  1. Healthcare Providers send a patient referral form to the NYS Quitline (either via fax or online) with the contact phone number for the patient interested in quitting.
  2. A Quitline counselor will call the patient (5 telephonic attempts are made) to counsel them by phone, screen them for 2 weeks of free NRT and set a quit date.
  3. A feedback report is sent to the referring provider that indicates what action was taken by the patient.
